Clinical Data for Care Coordination
CORHIO’s main focus is care coordination. We help providers improve clinical decision-making, which enables Coloradans to have higher quality healthcare. Better care coordination means better chronic disease management, fewer hospital readmissions and healthier patients.

Many accountable care organizations use PatientCare 360 to improve care coordination among the many providers their patients see. We provide organizations with their members’ real-time clinical data, which helps pinpoint areas for improving outcomes and containing costs. The prevalence of emergency department visits for adverse drug events in the United States was estimated to be 4 per 1,000 individuals in 2013 and 2014, and the most common drug classes involved were anticoagulants, antibiotics, diabetes agents, and opioid analgesics.
